EXCLUSIONS
The Membership Plan does not cover the following treatments and procedures:
Membership Plan savings do not apply to certain procedures that are not offered at our practice. In this case you may be referred to a Specialist who dos not participate in our plan.
Membership Plans are not (a) Insurance, (b) Qualifies under the Affordable Care Act, (c) Pre-payment for future services, or (d) Payment for access to discounted services. The Membership Plans are direct payment arrangements between plan members and providers for services.
Membership Plans are good for 12 months from the date of initial purchase. Plan members are responsible for renewals before expiration date, otherwise full cost will be applied to services completed.
Subscribed members are to preset future appointments that pertain to time frame of enrolment date and expiration. (12 months). We encourage our patients to pre-schedule, as availability can sometimes fill up quickly.
Once membership plans are active, there are no refunds or extensions. Cancellations fees are not excluded from membership plan. We require a 48hr business day notification for all cancellation/rescheduling in order of avoiding a cancellation fee.

Providers establish their own membership fees, treatment fees and terms. Fees and terms may vary depending on dental office, the services, the products and location.
The savings offered in the Membership Plan cannot be combined with any other offers or discounts.
The Membership Plan in not interchangeable between family and friends, nor are credit services applied for unused prepaid treatment.
The Membership Plan does not coincide with Dental insurance policies and clients with Dental Insurance are not candidates for this dental package discount.
